29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)	On or about October 4, 2023, at the machine shop,  the employer exposed employees to amputation/laceration hazards, in that  guards were not installed on the CNC Shapeoko/Makita milling machines (carbide3d1.8deg/step 2.0A(U) step2006).  b)  On or about  October 4, 2023, at the machine shop,  the employer exposed employees to amputation/laceration and injuries hazards, in that  guards were not installed on the  Swivel Metal Cutting Band Saws (Ser. No: 13793).

29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a)  On or about  October 4, 2023, the employer exposed employees to slips, trips, and fall hazards, in that flying chips and sparks HDPE High-Density plastic was on the floor, tables, and tools.

29 CFR  1910.22(a)(3):  The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.  a) On or about  October 4, 2023, between molding and storage room areas, the employer exposed employees to trip hazards, in that electrical conduit and electrical box installed on the floor protrudes approximately 2" inches, creating a tripping hazard.

29 CFR  1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s).  a) On or about  October 4, 2023, the employer exposed employees to health hazards when did not provide or had accessible Safety Data Sheets for the use of  4900-454G metal and others while soldering process in their working area.  b) On or about  October 4, 2023, the employer exposed employees to health hazards when did not provide or had accessible Safety Data Sheets for the use of High-Density Poly Ethylene (HDPE) while drilling, cutting, sanding, milling process in the Machine Shop.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  a) On or about  October 4, 2023, the employer exposed employees to health hazards, in that HAZCOM training was not provided for employees using various chemicals to include, but not limited to, Ease Release 200 (release agent sprays), Smooth-Cast Onyx (blank liquid plastic), EpoxAcast 650 (epoxy casting system), Mold Max 10 and 30 (silicone rubber compounding) and Smooth-cast (white semi-rigid urethane resin) while mixing and molding process in their working area.
