29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping or has guardrails which block employee access to the cantilevered end:   On October 3, 2023, employees of VYV Construction LLC were installing exterior siding onto a new residential home located at Lot 76 Pickney Ridge Estates in Warrenton, Missouri. The employer did not ensure a scaffold platform end 24 feet in length was positioned less than 18 inches over its support.  The walking platform on a pump jack scaffold extended 8 feet over its support exposing employees to fall hazards.

The employer did not ensure that employees working on a pump jack scaffold approximately 11.5 feet above the lower level were protected from falling by utilizing fall protection systems.  An employee was exposed to fall hazards to the ground below.
