Bridgeville, PA 15017 - On or about October 4, 2023, employees were working on a residential roof at heights up to 16 feet above the ground and were not utilizing fall protection.

Bridgeville, PA 15017 - On or about October 4, 2023, two employees were replacing a residential roof approximately 16 feet above the ground without utilizing some form of fall protection. The employer did not prepare a written certification record including the name or other identity of the employees trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.

Bridgeville, PA 15017 - On or about October 4, 2023, two employees were replacing a residential roof approximately 16 feet above the ground without utilizing some form of fall protection. The employer did not provide retraining to employees who demonstrated lack of knowledge and use of fall protection systems or equipment.
