Township Blvd., Pittston, PA 18640: On or about October 3, 2023, employees, performing roofing tasks, were exposed to fall hazards in excess of 28 feet due to using a ladder hoist that was not designed for personnel to climb to the roof of the building. b) Building rear, 900 N. Township Blvd., Pittston, PA 18640: On or about October 3, 2023, employees, performing roofing tasks, were exposed to struck-by hazards when hoisting materials on a ladder hoist due to the use of improper fasteners when constructing the hoist. c) Building rear, 900 N. Township Blvd., Pittston, PA 18640: On or about October 3, 2023, employees, performing roofing tasks, were exposed to struck-by hazards when hoisting materials on a ladder hoist, due to the lack of the use of a telescopic support brace, as required by the manufacturer, when the ladder hoist was assembled with over 36 feet of track. Abatement certification and documentation required.
