29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about 10/04/2023 at a residential construction job site located at 62 Sandwedge Drive, Saint Charles, MO., employees were exposed to a fall hazard. The exposed employees were observed by the CSHO climbing on the ladder rungs while the ladder was set up improperly.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles:  On or about 10/04/2023, Garcia Roofing, LLC was installing roofing materials to a residential structure located at 62 Sandwedge Drive, Saint Charles, MO. Employee(s) were observed operating pneumatic nail guns without any form of eye or face protection while working on a roof.  Garcia Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1563628, citation number 1, item number 1, and was affirmed as final order on 02/08/2022, with respect to a workplace located at 2901 Devondale Pl, Frontenac, MO.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 10/04/2023, at a residential construction job site located at 62 Sandwedge Drive, Saint Charles, MO., employees were exposed to a fall hazard. Employees were observed actively installing new roofing material, while working approximately 15 feet above the ground.   Garcia Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1563628, citation number 1, item number 2, and was affirmed as final order on 02/08/2022, with respect to a workplace located at 2901 Devondale Pl, Frontenac, MO.
