29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. Production Facility - On or about October 5, 2023, the employer did not ensure work areas were kept free of sawdust, debris and electrical cords. Excessive sawdust buildup in the facility, as well as scrap lumber and extension cords on the floor in work areas were creating fire, slip, and trip hazards for employees working in the facility.

29 CFR 1910.213(g)(1): Sliding cutoff saw(s) were not provided with an automatically adjusting hood that completely enclosed the upper half of the saw, the arbor end, and the point of operation at all positions of the saw. Truss cutting area - On or about October 5, 2023, the sliding cutoff saw by the Hundegger SC-1 machine was not adequately guarded, where the front blade guard was zip-tied in the up position and not guarding the blade at the point of operation.

On or about October 5, 2023, extension cords were not visually inspected before use where an extension cord was found to have a damaged outer jacket that had been taped and was in use at the facility. b.) On or about October 5, 2023, portable plug connected equipment was not visually inspected before use where the damaged plug of a power tool had been taped and was in use at the facility.
