29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about October 2, 2023, employees working in a trench measured to be four (4) feet or more in depth were not provided with a means of egress that provided less than twenty-five (25) feet of travel in any direction, exposing them to cave-in hazards.

29 CFR  1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  On or about October 2, 2023, a portable ladder with a broken rung was used by employees inside a trench as a means of access and egress, exposing them to cave-in hazards.

29 CFR  1926.651(e):Employees were not required to stand away from any vehicle being loaded or unloaded to avoid being struck by any spillage or falling materials:  On or about October 2, 2023, while backfilling operations were being performed, employees were inside the trench, standing directly where the backfill materials were being directed to be dumped, exposing them to struck-by hazards.

29 CFR  1926.651(f):A warning system such as barricades, hand or mechanical signals, or stop logs was not utilized when mobile equipment was operated adjacent to an excavation, when such equipment was required to approach the edge of an excavation:  On or about October 2, 2023, a front-end loader was being operated at and near areas of the trench edge where employees were inside working without a warning system in place, exposing them to struck-by and crushed-by hazards.

29 CFR  1926.651(k)(2): Where the competent person found evidence of a situation that could result in possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employee(s) were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety:  On or about October 2, 2023, the competent person did not remove employees from the inside of a trench once the development of fissures on the trench walls was observed and a protective system was not in use, exposing them to cave-in hazards.
