On or about October 5, 2023, at the job-site located at 22795 Cherokee Road Place, Land O' Lakes, Florida; employees using pneumatic nail guns while installing roof shingles were not wearing eye protection, exposing to struck-by hazards.

On or about October 5, 2023, at the job-site located at 22795 Cherokee Road Place, Land O' Lakes, Florida; employees were exposed to a 20- foot fall hazard while installing  roof shingles on the 6 in 12 pitched (vertical to horizontal) roof, in that, fall protection was not used.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a. On or about October 5, 2023, at the job-site located at 22795 Cherokee Road Place, Land O' Lakes, Florida; employees involved in residential roof work were exposed to a 10 and 20 foot fall hazard, in that, the multi-position ladders were not extended 3 feet above the landing surface of the roof.
