29 CFR  1910.147(c)(4)(ii)(B):The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:  a) On or about 10/05/2023 throughout establishment where  employees operate machines such as, but not limited to: CNC machines, horizontal band saws and the energy control procedures did not identify the magnitude of the energy sources and the specific energy isolating means for controlling energy were not identified. Also, generic statements such as "release any stored energy" and "go to the bus bar and pull disconnect off for the required machine" were used in the procedures without identifying whether there was a potential for stored energy and the means necessary for releasing the stored energy, or identifying the appropriate bus bar disconnect.  ABATEMENT CERTIFICATION REQUIRED

29 CFR  1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees:  a) On or about 10/05/2023 in the manufacturing areas; where employees performing servicing/maintenance on horizontal band saws did not unplug the machines(and control the plug) while performing servicing/maintenance such as changing the blade on the band saws. Also, the documented energy control procedures for some pieces of equipment such as, but not limited to CNC machines directed the authorized employee to "go to the bus bar and pull disconnect off for the required machine". The employees performing the servicing and maintenance were not utilizing the bus bar for deenergizing the machines.  ABATEMENT CERTIFICATION REQUIRED

29 CFR  1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) On or about 10/05/2023 in the parts wash area; where employees were exposed to GILLITE 0650, a corrosive chemical, in the parts washer. The SDS for the material states "flush with running water for at least 15 minutes". The only eye wash/drench facilities in the immediate area was a one liter squeeze bottle of saline solution.  ABATEMENT CERTIFICATION REQUIRED

29 CFR  1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) On or about 10/06/2023 in the machining area  by the MAZAK VTC-200C CNC machine; where standard duty metal, duplex receptacle boxes were used at the end of a flexible cord in a pendant application. The standard duty metal, duplex receptacle box is only approved for applications where the box is mounted in place.    ABATEMENT CERTIFICATION REQUIRED
