29 CFR 1926.503(a)(2)(iii):The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used: Worksite (61 Merrill's Wharf, New Bedford, MA): In the following instances, the employer did not ensure that employees had been adequately trained, as necessary, by a competent person in the use of personal fall protection: 1) When workers were conducting roofing activities in excess of six feet above ground level, the employer did not ensure that all exposed workers were protected from falls to the ground below. 2) The employer incorrectly used roof anchors as fall protection for workers. The employer allowed more than one worker to attach their lanyard to a single roof anchor, which negates the manufacturer's safe work practice instructions. 3) The employer allowed horseplay between two workers to occur on a pitched roof during roof shingling activities. Neither worker was connected to a lanyard during the horseplay, and this could have resulted in a fall to the ground below.
