29 CFR  1910.1450(e)(1): Where hazardous chemicals as defined by this standard are used in the workplace, the employer did not carry out the provisions of a written Chemical Hygiene Plan:  A)     In the laboratory, there was a fume hood that was not evaluated to make sure it could maintain a sufficient level of air flow as required by a Chemical Hygiene Plan.  Several chemicals were stored inside the hood, including a temporary plastic bottle with a hand mixed formaldehyde solution inside.  The employer did not have a Chemical Hygiene Program for the laboratory.  The Fish & Wildlife Service was previously cited for a violation of this occupational safety and health standard 1910.1450(e)(1), which was contained in OSHA inspection number 1624613, citation number 1, item number 4 and was affirmed as a final order on March 6, 2023, with respect to a workplace located at Longview, WA.
