On or about October 4, 2023, the employer exposed employees to fall hazards in excess of 4 stories while ingress & egress a scaffolding system in that the employer failed to conduct daily inspections of the scaffolding.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a.) On or about October 4, 2023, the employer exposed employees to falls from a 53' roof with a 6-12 pitch in that they did not ensure employees were in use of a personal fall arrest systems as they replaced roof shingles.
