29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.  Location: Blister pack area, machine BP4.  On or about October 4, 2023, Employees were exposed to the point of operation for the transfer head in machine BP4, where there was no guarding on the bottom section of the blister cup feed hopper. This condition resulted in an employee suffering a partial fingertip amputation when their fingers got pinched between the transfer head and the blister cup opening on the machine while attempting to clear a jam.
