29 CFR 1926.1404(h)(2): 29 CFR 1926.1404(h)(2):  The size, amount, condition and method of stacking the blocking was not sufficient to sustain the loads and maintain stability.   (a) On or before 10/7/2023, the employer did not ensure that the Elliott model 4000 truck crane was set up securely and properly leveled. The truck crane's outrigger was partially supported by the end of a curb and a rock bed with wood on top for cribbing that was bridging. The outrigger mud sill was not fully supported. This condition exposed employees and subcontractor employees to crane collapse hazards.

29 CFR  1926.1412(d)(1):A competent person did not begin a visual inspection of the crane prior to each shift the equipment was used or did not complete it before or during that shift:  (a) On or before 10/7/2023 the employer did not ensure that a visual inspection of the Elliott Model 4000 truck crane was conducted prior to use. The rigging was not tagged or inspected. The truck crane's outriggers were not set up correctly. This condition exposed employees and subcontractor employees to falling object and crane collapse hazards.

29 CFR  1926.1427(a): General requirements for operators. The employer did not ensure that each operator is trained, certified/ licensed, and evaluated in accordance with this section before operating any equipment covered under subpart CC, except for the equipment listed in paragraph (a)(2) of this section:  (a) On or before 10/7/2023 the employer did not ensure that the operator of the Elliott Model 4000 truck crane was qualified to operate the truck crane by being trained and certified as required by this standard. The employee was not accompanied by a supervisor o qualified operator. The operato was delivering trusses to a new single family home nder construction while subcontractor employees installed the trusses. These conditions exposed employees and subcontractor employees to falling object and crane collapse hazards.
