29 CFR1910.212(a)(1): One or more methods of machine guarding shall be provided to protect the operator and other employees from hazards created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Linen Department: On or about September 28, 2023 the employer did not ensure the stacker machine had adequate safeguarding to protect the operator from conveyor in-running nip and pinch point hazards resulting in an employee thumb fracture. b) Linen Department: On or about October 10, 2023 the employer did not ensure the folder machine had adequate safeguarding to protect operators from conveyor in-running nip and pinch point hazards at the rear of the machine. c) Linen Department: On or about October 10, 2023 the employer did not ensure the ironer machine had adequate safeguarding to protect operators from in-running nip and pinch point hazards at the rear of the machine.

29 CFR1910.264(c)(4)(iii)(a): All steam pipes that are within 7 feet of the floor or working platform in which an employee may come into contact with, shall be insulated or covered with a heat-resistive material or shall be otherwise properly guarded. Linen Department: On or about October 10, 2023 the employer did not ensure the ironing machine steam lines located to the rear of the machine ground level, were insulated, covered and/or guarded.
