29 CFR 1910.147(f)(1)(ii): Employees were not removed from the machine or equipment area in accordance with paragraph (e)(2) of this section during testing or positioning of machines, equipment or components thereof. At the mixing and dispensing area, employees were exposed to amputation, caught-in, and struck-by hazards. Employees engaged in end of day cleaning and shut down activities at the Mixer Drive Machine, were exposed to amputation, caught-in, and struck-by hazards in that the employees were allowed to enter the mixing drum and roller location to manually chock the roller.
