29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation: At the construction site located at 3166 N. Maize Rd., Wichita, KS, employee(s) engaged in masonry and plaster work of a new commercial structure who were utilizing a fabricated framed scaffold (tubular welded) were exposed to struck by hazards in that scaffold legs did not bear on any base plates or mud sills nor other adequate foundation. Scaffold legs were placed onto the ground and in areas where there was not firm foundation. The following were observed: 1. On the south side of the scaffold near the black entry door, on the first level, a scaffold leg has no base plate and is placed directly into the dirt with no mud sill. 2. On the south side of the scaffold near the black entry door, on the first level, a scaffold leg is on a base plate but no mud sill is present. 3. On the northeast corner of the scaffold, on the first level, a scaffold leg is on a base plate and a mud sill, but placed at the edge of a small trench, which is not an adequate or firm foundation.

29 CFR 1926.452(c)(2): All brace connections were not secured. At the construction site located at 3166 N. Maize Rd., Wichita, KS, employee(s) engaged in commercial masonry and plaster work were utilizing a scaffold whose brace connections were not secured. The following were observed: 1. On the north west corner of the scaffold, on the second level, horizontal cross braces were not secured or latched but tied in with metal wire. 2. On the south west corner of the scaffold, on the second level, horizontal cross braces were not secured or latched but tied in with metal wire. 3. On the south west corner of the scaffold, on the third level, horizontal cross braces were not secure or latched but tied in with metal wire. 4. On the south east corner of the scaffold, on the second level, horizontal cross braces were not secured or latched but tied in with metal wire. 5. On the north east corner of scaffold, on the second level, horizontal cross braces were not secured or latched but tied in with metal wire.
