29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: a) Roof: On or about 10/11/2023, the employer exposed employees to fall hazards of approximately 20-feet, 2-inches from the eaves of the 8/12 pitched roof, in that the lifeline was connected back to itself in the truss work by means of a knot as and anchor. b) Roof: On or about 10/11/2023, the employer exposed employees to fall hazards of approximately 12-feet, 8-inches from the eaves of the 8/12 pitched roof, in that the lifeline snap hook was connected back to itself in the truss work as the anchor. c) Roof: On or about 10/11/2023, the employer exposed employees to fall hazards of approximately 20-feet, 2-inches from the eaves of the 8/12 pitched roof, in that the lifeline tied in a knot as connection to the workers back. d) Roof: On or about 10/11/2023, the employer exposed employees to fall hazards of approximately 12-feet, 8-inches from the eaves of the 8/12 pitched roof, in that the lifeline tied in a knot as connection to the workers back.
