Employees conducting stucco activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: a) Jobsite at 216 Washington Ave, Downingtown, PA- On or about April 21, 2023, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the second level of the scaffold platform, thereby exposing employees to fall hazards.

The employee was utilizing a scaffold platform that did not have guardrail systems along the open sides and ends of the working platform, thereby exposing employees to fall hazards of approximately 15 feet.
