29 CFR 1910.22(a)(2): The employer did not ensure that the floor of each workroom is maintained in a clean and, to the extent feasible, in a dry condition. The employer did not ensure that when wet processes are used, drainage is be maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats are provided. On or about October 11, 2023, and at times prior thereto, in the Tower area, the walking/working surface was not maintained in a dry condition and mats and/or platforms were not provided. Employees were exposed to a slip and fall hazard from the wet conditions.

29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice. a. On or about October 11, 2023, and at times prior thereto, in the Tower area, employees were walking/working in an area where process piping protruded from the flooring. This exposes the employees to trip hazards. c. On or about October 11, 2023, and at times prior thereto, in the Tower area, employees were walking/working in an area where a flexible hose was placed along floor in front of a step. This exposes the employees to trip hazards. d. On or about October 11, 2023, and at times prior thereto, in the Tower area, the walking/working surface was not maintained in a dry condition. Employees were exposed to slip hazards.
