29 CFR 1926.102(a)(1):  The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about October 5, 2023, and at times prior thereto, employees were not provided eye protection while operating pneumatic nail guns to install roof decking, exposing them to eye injuries from flying objects.

29 CFR  1926.100(a):  Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about October 5, 2023, and at times prior thereto, the employer did not ensure that employees were wearing hardhats while working below other employees during roof decking operations, exposing employees to a struck-by hazard from falling objects.   Alianza Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1512378, citation number 1, item number 1, and was affirmed as a final order on April 5, 2021, with respect to a workplace located at 7834 Hoxie Lane, Frisco, TX, 75035.  Alianza Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1586034, citation number 2, item number 1-a, and was affirmed as a final order on May 2, 2022, with respect to a workplace located at 14132 Dorset Lane, Frisco, TX, 75035.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about October 5, 2023, and at times prior thereto, at a residential construction site, the employer did not ensure employees utilized fall protection while working at heights of six (6) feet or greater, exposing employees to fall hazards.  Alianza Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 1512378, citation number 1, item number 2, and was affirmed as a final order on April 5, 2021, with respect to a workplace located at 7834 Hoxie Lane, Frisco, TX, 75035. 29 CFR  1926.503(b)(1):  The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  On or about October 5, 2023, and at times prior thereto, where employees performing roof decking activities were exposed to fall hazards of up to 30 feet, the employer did not verify by written certification that employees received training in recognition and avoidance of fall hazards at heights over 6 feet to the ground below.
