29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire; a. On or about 10/12/2023, inside of 2-2-2 Estate Bonovi, St. Thomas, USVI Suite #105, three (3) oxygen cylinders were stored next to four (4) acetylene cylinders.

29 CFR 1910.1200(h)(1):Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets. a) On or about October 13, 2023, employees were exposed to hazardous chemicals, such as, but not limited to, Alkylbenzene and Cal-Blue, without training on where to access safety data sheets.
