29 CFR  1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):   On or about October 11, 2023, employees were performing residential construction activities from a multi-tiered scaffold system that was not fully planked, exposing employees to fall hazards.  Federico Gutierrez dba., FG Masonry  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1384052, citation number 1, item number 1 and was affirmed as a final order on May 13, 2019, with respect to a workplace located at 12616 Hanover Ct., Farmers Branch, TX.  Federico Gutierrez dba., FG Masonry  was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(b)(1), which was contained in OSHA inspection number 1591057, citation number 1, item number 2 and was affirmed as a final order on August 17, 2022, with respect to a workplace located at 12635 Mercer Parkway in Farmers Branch, TX.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  On or about October 11, 2023, employees performing residential construction activities were exposed to fall hazards when working on a multi-tier scaffold system without being provided with a safe means of egress.  Federico Gutierrez dba., FG Masonry was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 1384052, citation number one (1), item number two (2) and was affirmed as a final order on May 13, 2019, with respect to a workplace located at 12616 Hanover Court in Farmers Branch, TX.
