29 CFR 1926.652(a)(1): Employees in an excavation were not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a)Levanen Underground, LLC at Hospital Drive, Webster, SD 57274: On or about and at times prior to October 11, 2023, the exposing employer did not ensure employees were protected from cave-in and struck-by hazards when performing work in an excavation by use of an appropriate protective system. This condition exposed employees to cave-in and struck-by hazards.
