29 CFR  1926.451(b)(1)(i): Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  (a) All around building - On or about October 12, 2023, employees were working on scaffolding boards spread out from 6 inches to 3 feet in different places, exposing employees to fall hazards.

29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees:  (a) East and west side of building - On or about October 12, 2023, employees were standing on buckets and building material on top of 7-foot scaffolding to plaster higher levels of the building, exposing them to fall hazards.

29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  West side of building - On or about October 12, 2023, employees working on the ground did not have on hard hats protecting them from falling objects from 7 feet above where other employees were plastering on top of scaffolding, exposing employee to struck by hazards.

29 CFR  1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.  (a) West and east side of building - On or about October 12, 2023, the employer instructed his employees to use the scaffolding to plaster building without receiving training on how to use the scaffolding safely exposing them to fall hazards.
