29 CFR  1910.215(a)(2): Abrasive wheel(s) used on grinding machinery were not provided with safety guard(s) which covered the spindle end, nut, flange projections:  (a) at 6941 Upper Applegate Road, Jacksonville, Oregon , Recreation Bay area, on or about October 12, 2023 and at times prior thereto: Employees were exposed to pinch by and struck by hazard from rotating machinery parts of a table mounted fixed abrasive grinder wheel  missing side guards, resulting in extremities getting seriously injured or disabled.

29 CFR  1910.215(a)(4):On offhand grinding machines, work rests were not used to support the work and/or were not kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage.   (a) at 6941 Upper Applegate Road, Jacksonville, Oregon , Recreation Bay area, on or about October 12, 2023 and at times prior thereto: Employees were exposed to pinch by and struck by hazard from rotating machinery parts of a table mounted fixed abrasive grinder wheel  work rest was not kept adjusted maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage, resulting in body extremities or eyes getting seriously injured or disabled.

29 CFR  1910.151(b):First aid supplies approved by a consulting physician were not readily available:  (a)  at 6941 Upper Applegate Road, Jacksonville, Oregon , Fire House area, on or about October 12, 2023 and at times prior thereto: employees were exposed to skin injuries from expired first aid Antiseptic gel located in the fire house, for First-Aid use in the event of a exposure to burns, insect bites ,  poison ivy which could aggravate the employee's condition, as the expired solution could be a source of a potential skin infection or irritation.

29 CFR  1910.157(e)(2):Portable fire extinguishers were not visually inspected at least monthly:  (a) at 6941 Upper Applegate Road, Jacksonville, Oregon , Recreation Bay area, on or about October 12, 2023 and at times prior thereto: employees were exposed to potential fire hazard which could result in serious injuries or illness due to all portable fire extinguishers in the recreation bay area did not have  monthly maintenance inspections performed. Note:  Applies to any extinguishers provided in the workplace.  The Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1910.157(e)(2), which was contained in OSHA inspection number 1594183, notice number 1, item number 1a and was affirmed as a final order on8/18/2022, with respect to a workplace located at Forest Service, Parkdale Work Center.

Equipment:  Trailer ball hitch fork attachment  Condition: Employees were permitted to operate a forklift with attachment without approval of Manufacturer.

Equipment:  Man Basket attachment  Condition: Employees were permitted to operate a forklift with attachment without approval of Manufacture.
