On the roof of a temporary residential worksite, located at 1091 South McDuff Avenue, Jacksonville, Florida 32205: On or about October 11, 2023, the employer exposed five employees to eye injury hazards caused by airborne debris and flying objects, in that eye protection was not utilized by the employees, while utilizing pneumatic nail guns to install roofing shingles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by a guardrail system, safety net system, or personal fall arrest system, or an alternative fall protection measure that meets a provision described in paragraph (b) of this section: a. On the roof of a residential temporary home located at 1091 South McDuff Avenue, Jacksonville, FL 32205: On or about October 11, 2023, the employer exposed five employees to fall hazards, in that fall protection was not utilized by employees while performing roofing activities. Xtreme Labor, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1679072, Citation Number 1, Item Number 2, and was affirmed as a final order on October 3, 2023, with respect to a workplace located at 326 Naranja Avenue, Port Saint Lucie, Florida.
