29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions:  a) Jobsite, 333 Ella Street, Pittsburgh, PA:  On or about October 12, 2023, daily inspections of the excavation and the adjacent areas were not made by a competent person to identify the hazards that existed such as, but not limited to; lack of protection inside the 5.5' deep excavation, no safe means of access and egress, water accumulation, undermining existing structures, unsupported utility line and the spoil pile on the edge of the excavation.
