29 CFR 1910.1200(f)(6)(i): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information required by 29 CFR 1910.1200(f)(1)(i) through 29 CFR 1910.1200(f)(1)(v): The employer was failing to protect employees from chemical exposure hazards. This was most recently documented on October 12, 2023 at 806 West South Airport Road, Norfolk, Nebraska in the Paint Booth Room. Employees were using secondary containers not labeled after chemicals; such as, but not limited to: Chemlease 2710, Acetone, and Polishing Compound, were drawn from a source drum and used for longer than one shift.

29 CFR 1910.178(q)(6): Industrial trucks shall not be altered so that the relative positions of the various parts are different from what they were when originally received from the manufacturer, nor shall they be altered either by the addition of extra parts not provided by the manufacturer or by the elimination of any parts: The employer was failing to protect employees from struck by hazards. This was most recently documented on October 12, 2023 at 806 West South Airport Road, Norfolk, Nebraska in the Paint Booth Room. Employees were operating a walk behind powered industrial truck that had been modified by the employer to pick up and flip doors.
