29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  On or about September 22, 2023, operator(s) were performing cleaning of rollers while printing press 20 was running. Employee(s) were exposed to amputation hazards at ingoing nip points between the rollers. The employer did not develop, document and require printer operators to utilize machine-specific lockout/tagout procedures for the control of hazardous energy sources. Alternate protective measures, such as machine guarding were not provided.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
