29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems: a) Worksite, Apartment Roof - On or about October 14, 2023, two employees were performing work on the apartment building roof approximately 15-feet above the ground. The employees were not protected from falling.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) Worksite, Apartment Roof - On or about October 14, 2023, Employees were using an aluminum extension ladder to access the roof of a residential building approximately 15 feet above the ground. The side rails of the ladder extended approximately 2 feet above the landing surface.
