Employees were operating powered pallet jacks to move pallets of product throughout the facility without the benefit of being adequately trained. Due to this condition employees working on or around powered pallet jacks were exposed to struck- by injuries.

Due to this condition, employees operating forklifts and powered pallet jacks in the production and warehouse areas were not current on their performance evaluation, exposing employee working on or around the forklifts and powered pallet jacks to a potential struck by hazard.
