29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) On or about October 16, 2023, on the north side of the structure, employees worked without adequate fall protection while performing framing operations at heights greater than six feet above the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about October 16, 2023, on the north side of the structure, employees utilized a Franklin 17 ft Multi-Task Ladder to access the second level work area while the ladder was not extended three (3) feet above the landing.
