On or about 10/16/2023, the employer failed to inspect all electrical extension cords used to provide power to electric tools employees were using, in that a orange 100-ft. electrical cord was damaged and exposing the internal insulated energized wires. An inspection of these cords was not performed prior to the shift as these cords were not repaired or removed from service.
