29 CFR  1926.20(b)(2): The employer did not ensure that a system was in place to provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers:   a) Job site, 7815 McKnight Road, Pittsburgh, PA: On or about October 13, 2023, during construction activities, the employer did not ensure that a system was in place to identify and abate safety hazards. Deficiencies included, but were not limited to,  at least one employee working at a height of about 8 to 10 feet in an aerial lift and using a shock absorbing lanyard which would not protect him from hitting the ground if he had fallen from the basket.
