29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about October 17, 2023, the employer did not ensure that affected employees used appropriate eye protection such as chemical resistant glasses or goggles when exposed to liquid chemicals. Employees operating the compression mold press use chemicals such as, but not limited to, Filled EVA Barrier, 5283 64P, and Non-Woven Fabric, and do not wear eye protection.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and maintain at the workplace a written hazard communication program which describes how the criteria specified in paragraphs (f), (g), & (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes items (i) and (ii) of this subparagraph. On or about October 17, 2023, the employer failed to develop, implement, and maintain a written hazard communication program at the workplace. Employees were exposed to hazardous material to include, but are not limited to, Filled EVA Barrier and Uncured Glass Mineral Wool Insulation while operation the compression molding presses.

29 CFR 1910.1200(h)(1) :Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area. On or about October 17, 2023, the employer failed to provide effective information and training on hazardous chemicals in their work are at the time of their initial assignment.
