29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels and 50-feet (15.25 m) wide or less, was not protected from falling by guardrail systems, safety nets. (a) Roof: On or about 10/17/2023, and at times prior, the employer exposed employees to fall hazards of approximately 13-feet, 3-inches to the lower ground level, in that a fall protection system was not provided to employees engaged in roofing activities.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Roof: On or about 10/17/2023, and at times prior, the employer exposed employees to fall hazards of approximately 13-feet, 3-inches to the lower ground level, in that the ladder being used did not extend 3-feet past the working level while ascending and descending.
