(a) On or about October  17, 2023, the Werner Extension  ladder erected on the West side of the structure, at the garage, being used for access to the 8:12 pitched roof, did not extend at least three (3) feet above the landing edge, and/or was not secured to the structure when it was not sufficiently extended. This exposed employees accessing the roof  to a fall hazard of approximately 11 feet.
