29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  At the jobsite located at 5426 Sand Key Court, Wichita KS, 67227,employee using a Werner aluminum 20 foot extending ladder in order to access the roof was exposed to fall hazards of approximately 9.6 feet from the roofs' eave to the ground below in that the ladder was not extended at least 3 feet above the upper landing.

Employee(s) were utilizing the top step of a 4-ft. Werner A-frame ladder to access supporting wood framing for building  structure during construction activities

29 CFR  1926.501(b)(13): Employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system or personal fall arrest systems:   At 5426 Sand Key Court, Wichita KS, 67226,  employees engaged in framework were exposed to fall hazards of approximately 9.6 feet from the eave of the roof to the ground below.  The employer exposed the employees to these hazards by allowing them to work at heights greater than six feet without the use of appropriate fall protection  ELEPHANT FRAMING CONSTRUCTION LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1653057, citation number 1, item number 2 and was affirmed as a final order on June 13, 2023, with respect to a workplace located at 2807 Sunnyslope Street, Park City, Kansas, 67219.
