29 CFR 1910.219(c)(4)(i): Shaft ends projecting more than one-half the diameter of the shaft were not guarded by nonrotating caps or safety sleeves.  (a)  Resaw, outfeed at edger:  On or about, October 3, 2023, employees were exposed to a horizontal rotating shaft end that was approximately 3 inches in diameter and projected approximately 6 inches from the v-belt conveyer.  The shaft end was approximately 18 inches above floor level and was not protected with shaft end guard or cut off at 1/2 the diameter of the shaft end.  (b)  Resaw, outfeed to trimmer:  On or about, October 3, 2023, employees were exposed to a horizontal rotating shaft end that was approximately 3 inches in diameter and projected approximately 4 inches from the v-belt conveyer.  The shaft end was approximately 18 inches above floor level and was not protected with a shaft end guard or cut off at 1/2 the diameter of the shaft end.  (b)  Resaw, center of conveyor system above conveyor belt and electrical disconnect:  On or about, 10-3, 2023, employees were exposed to a horizontal rotating shaft end that was approximately 3 inches in diameter and projected approximately 4 inches from the v-belt conveyer.  The shaft end was approximately 4.5 feet above floor level and was not protected with a shaft end guard or cut off at 1/2 the diameter of the shaft end.  (d)  Resaw infeed at edger:  On or about, October 3, 2023, employees were exposed to a horizontal rotating shaft end that was 3 inches in diameter and projected approximately 6 inches from the v-belt conveyer.  The shaft end was approximately 4 feet above floor level and was not protected with a shaft end guard or cut off at 1/2 the diameter of the shaft end.
