29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about October 18, 2023 the employer did not ensure that effective machine guarding was in place on the following pieces of equipment to protect employee from the moving machine parts and components of them; a) The Doosan Puma 5100 CNC machine contained a door interlocking mechanism which did not function or was bypassed allowing the door to be open during production. Employees at times operate the CNC machine with the door open and running in order to remove material ribbon from the parts being machined. b) The Slant Turn 80n CNC machine contained a door interlocking mechanism which did not function or was bypassed allowing the door to be open during production. Employees at times operate the CNC machine with the door open and running in order to remove material ribbon from the parts being machined. c) The Doosan NHP 6300 CNC machine contained a door interlocking mechanism which did not function or was bypassed allowing the door to be open during production. Employees are not protected from the moving machine components in that the equipment is able to operate with the doors open. d) Mazak Nexus 8800 II horizontal CNC machine contained a door interlocking mechanism which did not function or was bypassed allowing the door to be open during production. Employees are not protected from the moving machine components in that the equipment is able to operate with the doors open. The front loading area contained an interlock device which allowed for table movement and rotation exposing employees to pinch points between the table and frame/ body of the machine. e) The Johnford JSV-45 CNC machine contained a door interlocking mechanism which did not function or was bypassed allowing the door to be open during production. Employees are not protected from the moving machine components in that the equipment is able to operate with the doors open. f) The Mazak 300 VTC CNC machine contained a missing panel/ door on the backside of the machine. This door acted as a barrier guard to moving machine components on the outside of the CNC machining interior. g) The Mazak FH-880 was not effectively guarded to prevent employees from reaching into the chip augers located on the lower portion of the non operator side of the machine.
