On or about October 17, 2023 at the address of 907 South Leonard Ave. Lima, OH 45804: The employer failed to ensure that the top step of a 6-foot step ladder was not used as a step. Employee(s) performing framing activities (installing stud walls) from the ladder in this condition were exposed to approximately 6 foot fall.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) Sandoval Framers Incorporated on or about October 17, 2023 at the address of 907 South Leonard Ave. Lima, OH 45804: The employer did not ensure employees were protected from falls while performing framing work activities (installing stud walls) atop the fourth level of a commercial wood frame structure. Employees installing wooden stud walls were exposed to falls up[to 34ft to the ground below. Sandoval Framers Incorporated was previously cited for a violation of this Occupational Safety and Health Standard 1926.501(b)(1), which was contained in OSHA inspection number 1657852, citation number 1, item number 8 and was affirmed as a final order on June 6, 2023 with respect to a workplace located at 2008 Homer Adams Parkway Alton, Illinois 62002.
