(a) On or about October 17, 2023, the employer did not utilize machine specific hazardous energy control procedures for the Jacob Hay Pasteurization machine to clearly and specifically outline the steps for shutting down and isolating the different energy sources. This exposed maintenance employees and passivation employees to pneumatic pressure, chemical pressure release hazards, and amputation hazards from mechanical energy.

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. (a) On or about October 17, 2023, the employer failed to train and instruct employees who are authorized to perform preventive maintenance and servicing of equipment to the authorized level. Passivation machine operation employees were trained to the affected employee level, but were tasked to perform filter change procedures that require lockout tagout procedures to be implemented.
