29 CFR 1910.178(m)(2): Employee(s) stood under or passed under the elevated portion of a powered industrial truck(s)  On or about January 23, 2024, employee(s) passed underneath the elevated forks of a powered industrial truck, while lifting an ACF Air Cool Flooded unit's frame in the assembly department to insert support beams underneath it. The frame was raised approximately 2ft off the ground and as a result the employee's right index finger was caught between the support beam and the frame.

The rated capacity of the sling was compromised and exposed employees to a hazard from suspended loads.   b) On or about October 17, 2023, a makeshift clevis comprised of bolts to hold the load was used on the 1/2Ton Gorbel overhead Gantry Crane located in the assembly area. The rated capacity of the shackle/clevis attachment was compromised and exposed employees to a hazard from suspended loads.
