29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a) Commercial building rooftop, various locations; Employees engaged in tasks associated with the replacement of an asphalt roofing system were not protected from falls while working at approximate heights ranging between 10-feet, 3-inches to 13-feet, 4-inches above ground level.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:  (a) Commercial building rooftop, various locations; The employer did not prepare a written certification record to verify training was received on applicable topics, not limited to, fall hazard recognition and mitigation procedures, for employees who were exposed to fall hazards at approximate heights ranging between 10-feet, 3-inches to 13-feet, 4-inches above ground level.
