Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard. Permanent aisles and passageways were not appropriately marked. On or about October 18, 2023, the employer did not ensure that aisles and passageways were not appropriately marked. These conditions exposed employees to struck-by hazards.

This condition exposed employees to struck-by and crushed-by hazards throughout the warehouse.
