29 CFR  1910.37(a)(3): Exit route(s) were not kept free and unobstructed:  a) Fabrication Building: On or about 10/18/2023, an egress path to a designated exit in the front of the building was impeded by carts with countertop slabs.  b) Fabrication Building: On or about 10/18/2023, an egress path to a designated exit in the back of the building was impeded by carts with countertop slabs and employee workstations.

29 CFR  1910.95(g)(1):The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  a) Fabrication Building: A Fabricator was exposed to continuous noise levels in excess of the allowable 8-hour time-weighted average sound level (Permissible Exposure Limit). The employee's dosimeter dBA level was 171.5% of the permissible level (90 dBA) and was equivalent to approximately 93.9 dBA. The sampling was performed for 435 minutes during one shift on 1/4/2024. Zero exposure was assumed for the unsampled period of time.

29 CFR  1910.179(j)(3): Complete inspections of the crane were not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section:   a) Fabrication Building: On or about 10/18/2023, the Spanco 1.5 ton hoist used to move countertop pieces was not inspected annually.  b) Fabrication Building:  On or about 10/18/2023, the Spanco 4,000lb hoist used to move countertop pieces was not inspected annually.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
