The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head. 181 West Town Street, Norwich, CT solar panel Installation project: On or about October 18, 2023, and at times prior thereto, employees lifelines were connected to the positioning D ring on the hip belt instead of the D ring on the upper back while performing solar panel installation activities from the Courtyard Marriot hotel roof exposing them to a fall hazard of 30 feet.
