29 CFR 1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a) Johns Manville on or about October 12, 2023, at the address of 408 Perry Street, Defiance, OH; The employer did not ensure that energy isolating procedures for the Glass Department; Furnace 2603 were implemented prior to employee performance of troubleshooting and maintenance activities. Employees performing troubleshooting and maintenance activities in an energized state were exposed to caught-in hazards.
