29 CFR 1928.57(a)(6): At the time of initial assignment and at least annually thereafter, the employer did not instruct every employee in the safe operation and servicing of all covered equipment with which he was or would be involved: (a) On or before 9/22/2023, the employer did not provide initial and annual training associated with the safe operation and servicing of equipment used by employees. This condition exposed employees to amputation, caught-in, and crushing hazards.

29 CFR 1928.57(a)(11): Where a moving machinery part presented a hazard during servicing or maintenance, the engine was not stopped, the power source was not disconnected, and all machine movement was not stopped before servicing or maintenance was performed, except where the Employer could establish the following in 1928.57(a)(11)(i-iii): (a) On or before 9/22/2023, the employer did not ensure that equipment was shut off and power disconnected when the potato conveyor system was being serviced by a vendor. The potato conveyor system was restarted and ran so the vendor could scrape a muddy roller under the belt. The vendor was caught in between the belt and the roller. This condition exposed employees and vendor employees to amputation, caught-in, and crushing hazards.
